Access custom fields in email before sending...

I have a custom email form that has custom fields in it. Once the email is sent, I can read the custom fields but I haven't figured out a way to read the custom fields before I send the email.

For example:

I have a custom field called "Matter Number"

When I press the Send button I want what ever was typed in "Matter Number" to be added to the subject of the email.
