iPrint Appliance user authentication issues

We have the iPrintAppliance up and running and printers migrated from an OES11 server. I'm having trouble getting logged in through the iPrint app. The IOS device says "invalid username or password" and the log shows "Error parsing user details from eDirectory". We set the authentication realm to our tree name (instead of the default IPRINT) and have the following:

LDAP attribute that uniquely identifies a user or group: GUID
LDAP attribute used for iPrint name: uid

The doc says to try setting up two LDAP entries to the same server. One with LDAP attribute used for iPrint name: cn and the other with LDAP attribute used for iPrint name: sAMAccountName

Just wondering if anyone has this working against eDirectory.
